New Patient Registration

docandcomputerTo register with the Practice you are required to call in and collect a registration pack for each member of your family. Contained within the pack will be a registration form and a short confidential questionnaire, which each patient will be required to complete. This questionnaire assists us in identifying any immediate medical needs that you may have. Please note we do not register patients on a Monday.

All newly registered patients are welcome to attend a health check within six months of their registration. This check includes a discussion on lifestyle and your general health. We will also undertake a blood pressure check and analyse a urine sample.
